When the patient first visited our Dublin clinic for a specialist orthodontic evaluation with Orthodontist Claire O’Sullivan, she presented with a combination of structural alignment issues and complex dental history:
- Blocked-Out Canine: Her lower right canine (tooth 43) was severely displaced buccally (sticking out toward the lip) due to arch crowding.
- Missing Molars: Existing space discrepancies due to previously missing back molars (teeth 16 and 36).
- Bite Misalignment: A Class II bite relationship with a lower midline shifted 3 mm to the left.
- Periodontal Baselines: Mild generalized gingivitis and localized plaque retention that required dedicated cleaning before fitting appliances.
A Customised, Phased Treatment Programme
Our clinical team design-engineered a non-extraction Damon metal brace programme to gradually expand the arches, align the blocked-out canine, and close the dental spaces.
1. Managing Orthodontics During Pregnancy (2024)
Early in 2024, during the active alignment phase, the patient shared the exciting news that she was expecting a baby! Pregnancy brings hormonal shifts—such as increased levels of relaxin and estrogen—which soften periodontal ligaments and often cause teeth to move more rapidly. However, it can also heighten soft tissue sensitivity.
Our team adapted her care immediately:
- Comfort & Ulcer Relief: At 20 weeks pregnant, when a protective bite-raising block on a back molar created a temporary tongue ulcer, Ortho Laura Herbsta promptly smoothed the material and prescribed safe, soothing chlorhexidine mouthwash and protective gel.
- Adaptable Chairside Care: When brief pregnancy dizziness occurred, our clinicians paused treatment, ensuring she was kept comfortable, supported, and relaxed throughout her appointments.
2. Finishing Alignment & Complete De-Bonding (Late 2024)
Following the safe arrival of her healthy baby in late summer 2024, the patient returned to finalize her smile line. With her teeth beautifully aligned into Class I canine guidance, the Damon braces were completely de-bonded in December 2024.
To hold her brand-new alignment securely, bonded retainers were placed behind her front teeth, paired with custom-moulded upper and lower clear Essix retainers for night-time wear.
3. Real-World Maintenance & General Dental Care (2025)
Orthodontic stability relies on real-world support. In early 2025, after eating chicken wings, the patient accidentally loosened a portion of her upper bonded wire. Dr Claire O’Sullivan acted swiftly, clearing the excess composite resin and adjusting her Essix retainer schedule to ensure zero movement occurred.
By late 2025, during a routine exam with general dentist Dr Daniele Ryan, updated bitewing X-rays confirmed zero dental decay, healthy bone levels, and excellent oral stability. Dr Ryan performed Professional Mechanical Plaque Removal (PMPR) and provided tailored diet and erosion prevention coaching to protect her enamel for the long term.
