When the patient first arrived at our Dublin clinic, they presented with a combination of advanced orthodontic and emotional hurdles:
- Severe Structural Crowding: The lower dental midline was shifted by 6mm to the right, with multiple teeth severely rotated or completely blocked out of the natural dental arch.
- Bite and Alignment Issues: A Class II division 1 relationship with an increased overjet.
- Extreme Dental Nervousness: The patient’s anxiety was initially so severe that standard clinical procedures could not be started without specialised, gentle management.

A Phased Framework: To manage the severe crowding, the plan required the removal of four premolars alongside the temporary placement of a Nance arch and a lingual arch to hold space and guide adult tooth movement.

By explaining every single step of the procedure beforehand and utilising guided breathing techniques, the team helped the patient successfully manage their panic, transforming their confidence in the clinical chair.

Restorative and Diet Coaching: During treatment, our general dental team (including Dr Stephen Taylor and Dr Daniele Ryan) identified early signs of enamel decay linked to high-sugar drinks. We provided tailored dietary advice—coaching the patient to swap sugary drinks for water between meals—and placed seamless, tooth-coloured composite fillings to protect the teeth.
Customised Solutions for Post-Treatment Comfort
After the braces were removed, the patient was fitted with custom upper and lower Essix retainers to maintain their perfect new alignment.
When the patient experienced temporary jaw clenching and TMJ discomfort a few months later, Dr Claire O’Sullivan promptly adjusted and cut back the terminal ends of the retainers. This allowed the posterior teeth to settle naturally, completely resolving the jaw pain within weeks.
The Result
Today, the patient has a fully aligned smile, a perfectly corrected bite, and significantly improved oral health. Most importantly, they transitioned from a patient who panicked in the dental chair to someone who confidently attends routine annual hygiene cleanings.
