The Initial Challenge: Congenitally Missing and Impacted Teeth
The patient first attended our practice as a teenager after being referred for specialist evaluation. A comprehensive clinical and radiographic assessment by Orthodontist Claire O’Sullivan revealed a complex matrix of severe structural and developmental hurdles:
- Congenitally Missing Lateral Incisor: The patient was naturally missing their upper right second tooth (tooth 12).
- Microdontia: The matching upper left lateral incisor (tooth 22) was naturally present but structurally microdontic (unnaturally small and misshapen).
- Severe Crowding & Impactions: Severe crowding across both dental arches, a palatally trapped premolar (tooth 15), and a severely impacted lower right second molar (tooth 47).
The Adaptive, Three-Phase Treatment Programme
To avoid a future titanium implant for the missing front tooth, Dr Claire O’Sullivan developed an ingenious, highly specialised orthodontic camouflage plan: we would close the front gaps by moving her sharp, pointed canine tooth (tooth 13) into the empty front space, later reshaping it to look exactly like a natural lateral incisor, while building up the opposite small tooth with composite resin.
1. Strategic Extractions & Advanced Space Management (2021 – 2024)
To resolve the severe crowding and create room to slide the canine forward, the patient was referred to our dental team for the extraction of three premolars. Following the comfortable extractions, fixed metal braces were bonded to both arches.
Over a three-year period, our team utilised specialised open coil springs and precise power chains to gradually shift the upper teeth, completely closing the midline gaps while deliberately preserving perfect, highly controlled spacing around the microdontic upper left tooth to prepare it for cosmetic enlargement.
2. Specialist Oral Surgery (Mid 2024)
During the alignment phase, the deeply trapped lower molar (tooth 47) began threatening the structural stability of the jaw. Dr Claire O’Sullivan immediately coordinated a specialist referral to oral surgeon Dr Deepak Thomas.
Under a comfortable general anaesthetic in Sligo, Dr Thomas successfully and surgically removed the impacted molar alongside a poorly positioned wisdom tooth. A post-operative evaluation just a few weeks later confirmed flawless healing with completely intact nerve sensation.
3. Premium Smile Design: Whitening & Cosmetic Bonding (Late 2024)
Once the braces were beautifully de-bonded, revealing an exceptionally straight arch wire alignment, the patient entered the cosmetic phase of her smile design:
- Bespoke Teeth Whitening: To achieve a bright, radiant baseline, the patient was provided with premium boutique whitening gels and custom trays. Our team carefully coached her to achieve a beautiful, bright shade.
- Bespoke Freehand Bonding: Once whitening was complete, general dentist Dr Daniele Ryan executed a non-invasive cosmetic transformation. Utilising an ultra-bright single shade (Venus Pearl BL), Dr Ryan performed freehand composite bonding without needing any local anaesthetic injections. She artfully sculpted premium composite resin over the small upper left tooth to give it a natural, full size, while meticulously contouring and rounding the sharp edges of the right canine to seamlessly disguise it as a natural front incisor.
Real-World Patient Care: Swift Management of Acute Conditions
In late 2025, during a routine retainer evaluation, our hygienist, Estelle Farrelly, identified that the patient had developed acute localised gum irritation (Acute Necrotising Ulcerative Gingivitis—ANUG) around her lower front teeth, making retainer wear painful.
Our team acted immediately with immense clinical safety. Estelle administered a gentle local anaesthetic, performed a precise localised root surface debridement (RSD) to clear the bacterial load, and prescribed an antibacterial chlorhexidine gel regimen along with specialised interdental cleaning coaching. This swift intervention successfully halted the condition, fully protecting her underlying bone and safeguarding her beautiful orthodontic result.
The Phenomenal Result
The patient completed her multi-year journey with a structurally flawless bite, an evenly balanced dental arch, and an exceptionally brilliant, radiant smile. By choosing a comprehensive specialist practice, this family completely avoided the need for highly invasive dental implants, finishing treatment with a beautiful, natural smile line that completely restored the patient’s self-esteem.
