The Initial Challenge: Congenitally Missing and Impacted Teeth

The Adaptive, Three-Phase Treatment Programme
To avoid a future titanium implant for the missing front tooth, Dr Claire O’Sullivan developed an ingenious, highly specialised orthodontic camouflage plan: we would close the front gaps by moving her sharp, pointed canine tooth (tooth 13) into the empty front space, later reshaping it to look exactly like a natural lateral incisor, while building up the opposite small tooth with composite resin.
1. Strategic Extractions & Advanced Space Management (2021 – 2024)
To resolve the severe crowding and create room to slide the canine forward, the patient was referred to our dental team for the extraction of three premolars. Following the comfortable extractions, fixed metal braces were bonded to both arches.
Over a three-year period, our team utilised specialised open coil springs and precise power chains to gradually shift the upper teeth, completely closing the midline gaps while deliberately preserving perfect, highly controlled spacing around the microdontic upper left tooth to prepare it for cosmetic enlargement.
2. Specialist Oral Surgery (Mid 2024)
During the alignment phase, the deeply trapped lower molar (tooth 47) began threatening the structural stability of the jaw. Dr Claire O’Sullivan immediately coordinated a specialist referral to oral surgeon Dr Deepak Thomas.
Under a comfortable general anaesthetic in Sligo, Dr Thomas successfully and surgically removed the impacted molar alongside a poorly positioned wisdom tooth. A post-operative evaluation just a few weeks later confirmed flawless healing with completely intact nerve sensation.
